By submitting this form you agree that a Ministry Team Lead for your department is aware and has approved for this volunteer or staff to receive a key/FOB. You understand that you or your Ministry Team Lead will be responsible for giving the key to the volunteer or staff person. Lastly, you will communicate with the volunteer or staff person that if the key/FOB is lost or not returned after the event that they will be charged a $15 fee to replace the key/FOB. The new key/FOB will not be issued until the fee is paid.

A copy of this form will be emailed to the Ministry Team Lead and appropriate admins.

Requests will be forwarded to our staff and members of our congregation who are part of the prayer team. If your request is confidential, please check the box and it will be forwarded to the Ministry Team only.
